Medical and Dental Leave
You may need to take time off during your working day for medical or dental appointments, but we encourage you to make appointments outside of hours that would be disruptive. Therefore, if you need to take time off during your working day, please try wherever possible to arrange appointments either at the beginning or the end of your working day.
This leave is unpaid. Alternatively, you should request holiday to cover these eventualities. If your dental/doctor's/hospital appointment is a planned attendance, you may be requested to provide appointment confirmation in writing.
NOTE: This leave type is applicable to you, our employee only. In the event that you wish to accompany another individual to a medical/dental appointment, you should request holiday.
Authorisation
Your Manager will approve your time-off. If leave is granted, it will be on an unpaid basis. In the event of emergency circumstances where you are unable to submit a Medical/Dental Leave form prior to your absence, please inform your Manager as soon as possible, and then complete the Medical/Dental Leave form upon your return to work.
